How do I apply for a Birth, Death or Marriage Certificate?

You may order these certificates online, through our UNIBank Online Bill Pay tab found on our Town Clerk page.
Mail In:
- Write the name and date of the record you are requesting.
- $10.00 check or money order payable to the Town of Warren.
- A self addressed stamped envelope.
